Position – Clinical Supervisor

Reports to – Chief Executive Officer (CEO)

POSITION SUMMARY:
TJH’s Clinical Supervisor position is clinically responsible for leading the direct care case management and harm reduction team in the management of Veterans’ Individual Recovery Plan (IRP) out of homelessness. This position is also responsible for the ongoing IRP program development of services for transitional facility residents. The duties and responsibilities require the Clinical Supervisor to take on the professional responsible in developing and maintaining a positive and working clinical relationship between TJH and VA medical and case management personnel. Additional clinical guidance will be used in this position to guide and support to our Veteran Outreach Team (VOT) in community outreach services. This position works collaboratively with TJH Facility Manager and the Business Administrator to ensure that appropriate facility policies and procedures supporting transitioning homeless Veterans are kept up to date and Facility Staff are trained and regularly in all transitional facility duties and responsibilities.

EXPECTATIONS AND SUMMARY OF ROLE

  • Responsibilities will include:
  • Is responsible for all harm reduction and clinical support given to Veterans transitioning out of the facility into permanent housing.
  • Lead the facility direct care case managers in the management of IRP development and outcomes.
  • Will provide clinical accountability for TJH processes involving:
  • Screening and acceptance of new referrals
  • Completion of psychosocial assessments, Veteran Individual Recovery Plans (IRP), and Veteran Safety Plans.
  • Participate in the orientation of new residents into transitional programs, resources, requirements, and consequences outlined in the Resident handbook.
  • Collaborating with VA Medical and Mase Management personnel on harm reduction issues and treatment referrals of all facility Veterans.
  • Provides brief solution focused harm reduction counseling, interventions and/or treatment support to TJH enrolled Veterans.
  • Establish and maintain a professional relationship with the VA and other community resources and networks through collaborative activities, and to be a positive example of TJH mission in these relationships.

  • Provides safe and clear clinical direction to case management team and facility staff in managing the harm reduction and transitional issues of homeless and at-risk Veterans.
  • Maintain a positive and collaborative relationship with VA and community partners of care.
  • Effectively manage the program development needs and processes supporting a Veterans transition into permanent housing without prompting or assignment.
  • Stay focused on Mission Strategies and Veteran Outreach Team (VOT) service standards when performing assigned duties and responsibilities.
  • Working effective as a team. Is professional in their passion to “end Veteran homelessness” and takes direction from supervisor or other team members.

DUTIES AND TASK OWNERSHIP:

  • Clinical Harm Deduction – Assessing the harm reduction dynamics and issues of homeless and at-risk Veterans within TJH mission programs.
  • Clinical Program Development – Oversee, update, maintain and develop evidence-based practices and decision-making procedures in transitioning Veterans into housing and/or independence.
  • Clinical Case Management – Oversee the clinical case management of Veterans transitioning into housing or housing independence, and /or into appropriate harm reduction treatment.
  • Community Relations and Veteran Outreach – This position will maintain a connection with the VA and other community resources and networks through outreach and networking activities for the purpose of finding and engaging homeless Veterans into the transitional facility.

SKILLS/QUALIFICATIONS:

  • Complex Problem Solving – Identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.
  • Judgment and Decision Making – Considering the relative costs and benefits of potential actions to choose the most appropriate one.
  • Communicating Effectively – Talking to others with respect and a high level of professionalism, with the ability to convey information effectively.
  • Critical Thinking – Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
  • Reading and Writing Comprehension – Understanding written contracts, grant language and the ability to construct sentences and paragraphs in work-related documents.

EDUCATION/CREDENTIALS:

  • Master’s Degree in either Clinical Social Work, Marriage and Family Therapy or Mental Health Counseling.
  • Holds clinical licensure of either Clinical Social Work, Marriage and Family Therapy or Mental Health Counseling.

KNOWLEDGE:

  • Case Management – Understanding of bests practices in social services case management, with accomplishments in managing complex and difficult processes in navigating others through lifestyle changes.
  • Communication – Understands the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Computers and Technology – Operates with basic understanding of computer programs of Microsoft Words Suites, Electronic Record Management (ERM) systems, phones, email, and internet.
  • Public Safety and Security – Knowledge of relevant equipment, policies, procedures, and strategies to promote effective local, state, or national security operations for the protection of people, data, property, and institutions.
  • Customer and Personal Service – Understands principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
